The e-smith server and gateway normally works in "dial-on-demand" mode, using the modem as needed to simulate the illusion of being permanently connected to the Internet.
The dial-on-demand heuristics are oriented to the North American market where unlimited local dialup access is the norm. For example, if you visit a web page, the line is kept up for 20 minutes so that it's not going up and down as you surf. If you are in an area where calls are very expensive and you are comfortable using Linux, you can customize the e-smith server to minimize the amount of phone-calling (although e-smith doesn't offer support for this).

No, we don't support ISDN cards. Only analog modems and ethernet connections. So our ISDN customers typically purchase routers such as the Ascend Pipeline 50, which connect to the e-smith server via ethernet.

Yes, the email system has several modes of operation, including "multidrop" where all mail for the domain is fetched from a single POP account.
