The e-smith server and gateway in server-only mode only has one active network card. You will need to be able to reach the internet via the gateway IP address for the server to be able to send internet email, unless you use the SMTPSmartHost feature, and relay all outgoing mail via another server.
I don't understand your network configuration as I don't know the configuration and features of the proxy server. The e-smith server and gateway was not designed to work in such a configuration. It was designed to have a direct connection to your ISP via the external interface. If you cannot operate it in that configuration, then you will need to work out exactly what is going wrong with your setup. Is there a routing problem? Is there a name resolution problem? Is the proxy server blocking incoming and/or outgoing connections?
Best wishes
