Title: Adding second external interface

I want to add my SME 6 server third ethernet card acting as a second external interface. I want one internal two external interfaces. When I add extra interface as described in this link, does it become an external interface if I will not define it as internal. If it is possible, do I need extra configuration to respond smtp and http requests through that second external interface.

If you add another interface, it will be whatever you make it.  If you put in a private IP, it will be private, public IP - public or external.  Remember, if it is external, you need to configure a firewall rule set for the new interface.  If not, you will leave your machine open for attack.
