I got a call from one of the SME servers we deployed 2 years ago or so. It is a 6.0 version and recently ( last several months ) they have been having slowness preblem when sending email. Last I heard from when they click send on a message till the server accepts it takes about 45 second to a minute. I don't see anything in the logs that indicates this. I first suspected that it was thier workstation antivirus but the turned off outgoing email scanning and that didn't change the speed at all. I have knudens antivirus and spamfilter on it and I turned those off temporaily and that didn't change anything speedwise either.
I'll take any suggestion at this point.

Possibly RBL list lookups for your inernal IP, reduce the number of RBL lists to max 4.
This has been discussed in these forums.

I think there is something in the contribs.org bug tracker about a similar issue (search).

Reducing the RBL list to 4 did indeed take care of the problem.

Some time ago RBL ipwhois.rfc-ignorant.org was removed (see www.rfc-ignorant.org for more details). If someone still uses this RBL, you will experience delays such as your customer did.
Personally I use the following RBLs without problems:
abuse.rfc-ignorant.org
bogusmx.rfc-ignorant.org
dsn.rfc-ignorant.org
postmaster.rfc-ignorant.org
relays.ordb.org
sbl-xbl.spamhaus.org
