To me it looks you are trying to send e-mail for a domain that is not configured on your SME Server. As a safety (anti-spam) measure SME Server is configured not to allow relaying as to prevent your machine from being used to send SPAM.

Thank you for you quick reply.
The xxxx@3xgmm.com user and domain is configured on the SME server and has been used for 2 years. xxxx@3xgmm.com can send email to other email addresses without problem.
The xxxx@dokom.net is the user and domain that xxxx@3xgmm.com is trying to send email to via Outlook client.

I believe I have solved the problem
It appears that the issue was with Outlook and authentication.
After clearing login information, closing outlook, logging off, logging back in and reconfiguring account login information in outlook.
The problem now appears to be resolved and emails are being delivered to addresses that previously were getting relayed denied.
The odd thing was that emails sent to users setup locally on the SME server domain (user@3xgmm.com) were being delivered, however external domains were randomly being denied.
If the outlook smtp login details were wrong then no emails should have been delivered at all, because smtp sme server should have rejected users connection ?
Users computer and SME server are NOT local or connected through local interface.
I will report back if the problem occurs again.

No, that isn't true. Email for valid local recipients is accepted from anywhere, from anyone, without authentication. That's how email normally gets from place to place.
