Title: IVR broke on upgrade from 2.2 to 2.3 version of SAIL

Just noticed since I upgraded a week or so back that the IVR's I have that end on dropping to a call group now just continuosly repeat the greeting. It was an easy fix to change the timeout outcome to fix it, but it maybe a gotcha for someone.

The other IVR's that do a hangup are still fine..

As I am not sure if it was something on my box that caused the issue in the first place, it was just a matter of restting the ivr options manually.

It was more a waring incase I wasn't the only one who couldn't figure out why my ivr stopped working after the update..

Also, IVR is not listening to extension dialed. It responds only to item in the IVR menu.
It seems not possible to have "if you know the extension, dial it now... or follow these instructions..."

It seems that IVR menus are mixed with Greetings.
I created an IVR associated with a greeting but the trunk lets me select greetings, not the IVR are just created.

Yes they are and yes we do show the greeting number rather than the IVR name in the drop down, which results in apparently poor entity integrity (entity integrity is, nontheless, maintained via the file hash).  Its basically lazy programming because it saves us a bit of cpu figurong out the greeting.

Could do better...   
I'll put it on the fix-list.
