my bandwidth around 3mpbs
in upload?

... it just keep in the outbox...
If it's just in your Outlook Outbox, it has never been (successfully) sent to the SME server.
Look in /var/log/qpsmtpd/current on the SME server (or possibly /var/log/sqpsmtpd/current) to see what's holding it up.

1. your SMTP mail server is you SME box?
if you are in LAN side did you tried to put your internal (LAN IP of SME box) as SMTP server?
I saw this behaviour if SMTP was from your ISP provider and you don't use your server as relay
2. if you use your SME box as relay
what kind of configuration you use ?
(relay local only, relay from whole internet - what ports 25, 465...)
-
my upload is 1mbps,
smtp server is my sme, i use port 465.
log for /var/log/sqpsmtpd/current

5 minutes have the email still in the outlook 2007 outbox...
15 minutes later email reach its destination but still in my outlook2007 outbox?

Try to isolate the problem.
My first tip is to use Thunderbird to send same message.
If it's a Microsoft problem, in TB you'll have a quick send.
Other possible problem is a tarpit... maybe your domain is blacklisted and Gmail is slowing down your SMTP (if internal). Try to send same message using another account (a gmail would be perfect) to same destination.
Post results here.
