att.net on a static IP DSL imposes severe restrictions on outgoing mail,
it seems like the limits are 1 message/minute, no more than two recipients per message.
Here is a typical log file message for a deferred email, from the file /var/log/qmail/current
2012-08-29 23:15:35.720573500 delivery 227631: deferral: Connected_to_127.0.0.1_but_greeting_failed./Remote_host_said:_451_Upstream_SMTP_server_not_available/
The downside is that if an email is sent to a large group, or list, the deferrals get extended and messy. Some messages may never be sent.

however it seems these solutions dont work anymore.

Get a proper ISP. The S stands for Service.

Use a carrier running on food pellets rather than switched packets? (Internet Service Pigeons)

I doubt that you will find a workaround. From what you have told us, the problem isn't qmail related, but restrictions on the SMTP protocol imposed by the I[no]SP. If so, it doesn't matter what SMTP client software you use (qmail, serialmail, maildirsmtp, postfix, sendmail, ...) you will hit the same limits.

the best you can do is : switch to a real ISP.
the best workaround : pay for an external smtp service ( if your InoSP allows you to do so)
the alternate give it a try :
config setprop qmail ConcurrencyRemote 2
signal-event email-update
it won't limit the number of connection per minute, but at least you will send only 1 to 2 mails at a time instead of 20 as a default.
i have seen some ISP smtp limitations but 1 mail /minute is really a no go!

For a modern carrier to specify that criteria in writing is amazing.
So, use the specification (against ATT) by taking it to an alternative
carrier's marketing department and challenge them to do better.
Offer them the opportunity to take a swing at ATT whilst actually
using an honest fact ...ie that ATT are THAT bad for business.
If you've only a single carrier then take it to a watchdog organisation.
