I have a server running SME v 5.0, and would like to update it to v 5.1.2. At the same time I am also going to replace the hardware with a faster machine running SCSI disks. So my questions is: How should I proceed to upgrade the software/hardware? What steps should I take? Will I lose all the add-ins and customizations on my current v 5.0 server?

Depends on what add-ins and customizations you've done.
In general:
Step 1: Upgrade your current server1 from 5.0 to 5.1.2 and make sure everything works.
Step 2: Install a fresh 5.1.2 install onto your new server2
Using the same or idenitical TBU
Step 3: Perform a tape backup on server1
Step 4: Perform a tape restore on server2
Step 5: Reload all your add-ins
Step 6: Review and repeat any customizations as needed
