Boot failed

I am attempting to upgrade from 4.0.1 to the new version 4.1.2 and are having problems doing so.
The PC I am using is and old IBM PC server 310 and it does not have a bootable CD-ROM, so following the instructions I made a boot disk, 2 of them in fact one using the linux commands, the other under windows.
Using both of them I get the same message when attempting to install the new version,
It boots from the floppy and starts loading :-
"Loading initrd.img"  and the the message
"Boot failed : please change disks and press a key to continue"

And it then it loads the old version  !!!!

The system is the same that I installed the older version onto using the boot floppy disk so hence I have no idea what is wrong.

If anyone can help it woild be appreciated

If you boot from floppy disk, the disk needs to have zero errors. Freshly formatting the disk in the target system is a good idea. The command to do that on the e-smith system is:

fdformat /dev/fd0H1440

Thanks for your help and advice, It worked after reformating and recreating the floppy.
So I am know running on v4.1.2 and are very happy, the upgrade option worked fine.
