I added a LPT card (ISA) recently. I found the card in a junk pile, with no documentation or driver software I finally got it working by doing the following:
In the PC's BIOS setup, I changed the IRQ and memory address of the on-board LPT1 interface to something other than the default. By trial and error I finally hit a setting that made both printer ports visable to e-smith.
The default memory address on the LPT card was set to same as the BIOS default for the onboard port. In the end, it was only the memory address setting that required modification, the IRQ stayed at the BIOS default.
Does the card have any jumpers that let you set the IRQ and Memory Address? If so, choose something that will not conflict with your other devices.
