Spam Assassin and Forwared email

I have 5.6 set up in front of my exchange box and what I was thinking about implementing is a spam filter smtp gateway. It all works just fine as far as smtp traffic but one thing I don't see working is spam assassin. For some reason it looks like forwarded email messages are no concern to spam assassin.
Is there something I am missing here? Has anyone had experience with a similar setup?

Spam Assassin, as set up by the SME RPMs, only filters messages for local delivery, AFAIK.  Don't know a way around this, though I'm sure it's possible.

As Dan said, by default it uses procmail which requires user accounts.
The ultimate would be to not have to create user accounts on SME and have all processed mail delivered to your exchange box.

If you create a procmail rule for each user you can send a copy of the message to an email address.

As fas as I can see this then gives you the similar functionality to the "deliver locally and forward" setting in user accounts (ie when not using procmail).
The message is therefore filtered by Spamassassin before forwarding externally.

Does someone else agree with that ?

I thought of your idea of setting up procmail rules for each user. That would not be such big issues for I only have 70 users, but what do I do with the mail that remains on the server?
