Topic: Problem changing user password

If I get a user to try to change their password through

http://server/e-smith-password

when they press return to enter the new info:

Software error:
Failed to set Unix password for account mike.

OR

Page not found.

Any ideas why? I'm using SME 5.0 update 2.
