I've been running 7.1.3 fine for over a month but am getting "an error occured adding the user" when I try to add a user through the server panel.
I cannot recall making any changes to the server or installing any contribs that could have caused thing. Please can someone shed light on this? I get no error is /var/log/messages when adding a user but I do get these warnings.

I don't think this is a bug as everything was fine before this week.
Also I don't know where it's getting the "another" from as I'm not entering this anywhere when adding the user.
